Pancoast syndrome is a rare type of lung cancer that occurs in the apex of the lung, the area where the lung meets the collarbone. It is caused by a tumor that invades the nerves and blood vessels in the chest, leading to a variety of symptoms, including pain in the shoulder, arm, and hand, weakness in the arm and hand, and Horner's syndrome, which is a drooping eyelid and constricted pupil.

Diagnosis

Pancoast syndrome is diagnosed based on a combination of symptoms, physical examination, and imaging tests. Symptoms that suggest Pancoast syndrome include:

  • Pain in the shoulder, arm, and hand
  • Weakness in the arm and hand
  • Horner's syndrome
  • Cough
  • Shortness of breath
  • Weight loss

Physical examination may reveal Horner's syndrome, weakness in the arm and hand, and decreased sensation in the arm and hand.

Imaging tests that can be used to diagnose Pancoast syndrome include:

  • Chest X-ray
  • Computed tomography (CT) scan
  • Magnetic resonance imaging (MRI) scan

Treatment

The treatment for Pancoast syndrome depends on the stage of the disease. Surgery is the primary treatment for early-stage Pancoast syndrome. The goal of surgery is to remove the tumor and any affected nerves and blood vessels. Radiation therapy and chemotherapy may also be used to treat Pancoast syndrome.

Surgery

Surgery for Pancoast syndrome is a complex procedure that requires a team of experienced surgeons. The type of surgery performed will depend on the location and size of the tumor. In some cases, a lobectomy, which is the removal of a lobe of the lung, may be necessary. In other cases, a pneumonectomy, which is the removal of an entire lung, may be necessary.

Radiation therapy

Radiation therapy is a type of cancer treatment that uses high-energy beams of radiation to kill cancer cells. Radiation therapy may be used before surgery to shrink the tumor or after surgery to kill any remaining cancer cells. Radiation therapy can be delivered externally, using a machine that directs radiation beams at the tumor, or internally, using radioactive seeds or pellets that are placed inside the tumor.

Chemotherapy

Chemotherapy is a type of cancer treatment that uses drugs to kill cancer cells. Chemotherapy may be used before surgery to shrink the tumor or after surgery to kill any remaining cancer cells. Chemotherapy can be administered intravenously, through a vein, or orally, by mouth.

Prognosis

The prognosis for Pancoast syndrome depends on the stage of the disease at the time of diagnosis. The five-year survival rate for early-stage Pancoast syndrome is approximately 60%. The five-year survival rate for late-stage Pancoast syndrome is approximately 20%.

Prevention

There is no sure way to prevent Pancoast syndrome. However, quitting smoking and avoiding exposure to secondhand smoke can help to reduce the risk of developing lung cancer.
